An Advance Health Care Directive, or Living Will, allows you to appoint an agent to make health care decisions for you, if you cannot make them for yourself. In the event that you are physically or mentally incapacitated, your agent will be able to make health care decisions for you.

Directives allow you to specify the type of treatments you may or may not want, and allow you to limit your agent’s authority on those treatments. Directives include organ donation, disposition of remains, and an End of Life Decision.
